Physicals are due BEFORE the first day of practice. Please do not bring the physical to the first day of practice.

IMPORTANT- please print and fill out this form and bring it to your physical appointment. They will NOT have forms for you there and it must be filled out before you arrive.

Please bring your completed form to either of the parent meetings in the spring (date TBD), or if you'd like to get them in earlier, you can mail them to:

If your athlete got a physical last season, you must get a new one, as your child's physical must cover the entire season, July-December.
